All meals will take into consideration your restrictions and allergies.

Specify:

a) whether you need to split the items into different tax entities;
b) whether you want the invoice to combine all the parcels into a single item (Full Entry);
c) whether the invoice should include several participants.
Indicate any specific invoicing requirements that are not covered in this form, such as description, order number and other elements.

Only needed if your institution requires a purchase order or internal reference number on the invoice. Leave blank otherwise.
